On January 17, 1979, the crew of CG-1376, an HH-52A helicopter, were descending into the Opa Locka Airport near Miami, Florida, when the aircraft was involved in a midair collision with a civilian helicopter in a takeoff climb. Both aircraft had been cleared by the tower along parallel flight paths, but were not advised of each other. On board were AM1 R. McClain, AD3 J. Case, LT R. Shearer, Jr. and LT R. Ausness. There were no survivors.
