On November 26, 1969, CG-1458, an HH-52A helicopter based in St. Petersburg, Floria, was ordered to search for an overdue boat with two elderly fishermen onboard. Approximately 25 minutes into the search, Air Station St. Petersburg received a “Mayday” call from the helicopter, then silence. The Air Station launched several other assets to search for the missing rescuers and eventually located wreckage at around midnight. On Thanksgiving Day, the Coast Guard Buoy Tender Juniper salvaged the wreckage and recovered the helicopter crew’s bodies, including that of Aviation Machinist Mate D. A. Lumsden.
It was thought the aircraft had a loss of a critical flight component, lost control then impacted the water.
The fishermen had safely returned to shore earlier, but failed to notify anyone they were back.
